Community Service Form

Here is the community service form for students to fill out for community service that they completed between April 1, 2017 and March 31, 2018. They need to fill in what the service was that they completed and where they did it, the amount of hours completed, and someone from the organization, event, church, etc. that they completed this service for needs to sign the form. Fund raising activities for a class or school organization or court-ordered community service do not count.
Students who complete 30 hrs. or more will receive a gold medal, students who complete 20-29 hours will receive a silver medal, students who complete 10-19 hours will receive a bronze medal, and students who complete 1-9 hours will receive a certificate.
All forms are due to Mrs. Reuter by Friday, April 6, 2018. Late forms will not be accepted.
